Immunization Requirements
Minnesota law mandates that children enrolled in a school program be current with specific immunizations, or provide a legal exemption to the school. Failure to comply with immunization requirements or provide exemption documentation may result in exclusion from school. View the Immunization Requirements form below to see if your child is up to date. If you have any questions regarding your child's immunization status, please contact the school nurse's office. Immunization Requirements Immunization Exemption Form Medications in School Over-the-Counter (OTC) and prescription medication may be administered at school. Per district policy the following documentation must be provided before any medications may be administered. OTC authorization completed and signed by the parent for any non-prescription medication administered during school hours. Over the Counter Medication Form Prescription authorization completed by and signed by the health care provider and parent for any prescription medication administered during school hours. Prescription Medication Form Medications must come in the original container, and must not be expired. It is the responsibility of the parent or legal guardian to provide the medication to the school. Medications will be administered out of the school health office by designated, trained staff. Are you new to our district? Do you have young children not yet enrolled in our school? It is important that we have your family's information on file so we can stay in touch about your child's education. We partner with Mid-State Education District to provide Early Childhood Screenings. If your child has not been screened, please contact Beka Swisher at 320-631-2509. SMART SNACKS! The Alliance for a Healthier Generation provides a smart snack calculator on their website to help students and parents determine if the snacks they are eating are considered healthy. |
