CHROMEBOOK SUMMER STORAGE AND REPAIRS

1. Students in grades 5-11 will take their Chromebook home for the summer, returning to school with their FULLY CHARGED Chromebook on the first day of school in the fall. However, if you feel you would rather store your student's Chromebook at school, please fill out this form before Tuesday, June 1.

2. For any repairs needed over the summer, please fill out this form, and place your Chromebook in the black tub inside Door 2 at the high school. 5th and 6th Grade: STEP-BY-STEP GUIDE FOR THE 2021-22 SCHOOL YEAR:

  1. Read the Chromebook Policy & Discipline Policy.
  2. Complete the Chromebook Policy Signature Page.
  3. If purchasing the optional insurance, all payments can be made online (after August 15, 2021). Insurance rates take into consideration the year/make/model of each grade's Chromebook. If you've paid in the past for multi-year coverage, you are still covered
  4. 5th Grade::  If purchasing the optional insurance,  pay the $89 fee online here which will cover the Chromebook until the end of 8th grade.  This covers unintentional damage to the Chromebook for 4 years as outlined in the policy page.
  5. 6th Grade:  If purchasing the optional insurance,  pay the $69 fee online here which will cover the Chromebook until the end of 8th grade.  This covers unintentional damage to the Chromebook for 3 years as outlined in the policy page.
  6. IF YOU ARE UNABLE TO PAY ONLINE, YOU CAN SEND CASH/CHECK with your student in an envelope marked with student name and 'Chromebook Insurance'.  Checks payable to ISD 484.
  7.  Students will be able to go home with the Chromebook, protective bag, and charger once the above form is filled out and insurance (if you choose to take out insurance) is paid for.  Students not taking out the optional insurance may still take Chromebooks home, provided the appropriate grade-level signature page (see above) is filled out as such.  Chromebooks will not be going home over the weekends or holidays.

Students need to fully charge their Chromebook before coming to school on Tuesday, September 7th.
